By accessing or using the Continuity mobile application ("Continuity", "we", "us", or "our"), you agree to be bound by these Terms of Service.

If you do not agree to these terms, you should not use the app.

2. Purpose of the App
Continuity is designed to support individuals in tracking their mental health care, including sessions, reflections, and wellbeing check-ins.

Continuity is not a healthcare provider and does not provide medical or psychological advice.

3. Not a Substitute for Professional Care
Continuity is intended to complement, not replace, professional care.

You acknowledge that:

The app does not provide diagnosis, treatment, or crisis support
You should seek support from a qualified professional where needed
In an emergency, you should contact appropriate services (e.g., 000 in Australia)


4. User Responsibilities
By using Continuity, you agree to:

  • Provide accurate and up-to-date information

  • Use the app for personal, non-commercial purposes

  • Maintain the confidentiality of your account login details

  • Not misuse the app or attempt to interfere with its functionality


5. Your Data
You are responsible for the information you enter into the app, including:

  • Session notes

  • Check-ins

  • Uploaded documents


You acknowledge that:

The app stores personal and sensitive information
You should avoid entering information you are not comfortable storing digitally

6. Use of Artificial Intelligence (AI)
Continuity may use artificial intelligence (AI) features to assist with processing information you provide.

This includes:

Automatically extracting and populating information from uploaded documents such as referrals, treatment plans, and invoices
Generating summaries or structured data based on uploaded content


You acknowledge that:

AI-generated outputs may not always be accurate, complete, or up to date
You are responsible for reviewing and confirming any information generated by the app
Continuity does not guarantee the accuracy of AI-generated content
AI features are designed to assist with organisation and efficiency and should not be relied upon as a substitute for professional advice or verification


7. Medicare Disclaimer
Continuity is not affiliated with, endorsed by, or connected to Medicare or any Australian government agency.

While the app may assist users in tracking sessions that are eligible for Medicare rebates (such as under a Mental Health Treatment Plan), it:

  • Does not submit claims to Medicare

  • Does not provide official records for Medicare purposes

  • Does not guarantee eligibility for rebates

  • Users should consult Medicare or their healthcare provider for official information regarding eligibility, claims, and benefits.

8. Availability of the Service
We aim to ensure Continuity is available at all times, but we do not guarantee uninterrupted access.

We may:

  • Update or modify the app

  • Temporarily suspend access for maintenance

  • Change features or functionality over time


9. Fees and Payments
Some features of Continuity may require a paid subscription.

By subscribing, you agree that:

  • Fees will be charged as disclosed within the app

  • Subscriptions may renew automatically unless cancelled

  • You are responsible for managing your subscription


10. Intellectual Property
All content, design, and branding within Continuity are owned by Connected Minds Pty Ltd or licensed to us.

You may not:

  • Copy, reproduce, or distribute the app content

  • Reverse engineer or attempt to extract source code

  • Use the app for commercial purposes without permission


11. Limitation of Liability
To the extent permitted by law, Connected Minds is not liable for:

  • Any reliance on information within the app

  • Loss of data or interruption of service

  • Any direct or indirect damages arising from use of the app


12. Termination
We may suspend or terminate your access to the app if:

  • You breach these Terms

  • We are required to do so by law

  • The app is discontinued

  • You may stop using the app at any time.

We may update these Terms from time to time. The latest version will be available within the app. Continued use of the app constitutes acceptance of any updates.
